Registration Form/Profile

I need to add more fields on both of these. How?
a) Field names I need to include are
Company Name
Town
State
Country
Email
Cell Phone
Website
b) when user registers this automatically creates wordpress user yes? but where do these requests show up in admin – under users/pending? or? i’m just a little unclear on the logic behind this. We need to approve each request and can’t ascertain where the requests go?

    a) Take a look to Affiliates Extra Fields
    b) Affiliates are automatically accepted. If you need to have a restricted affiliates system, you can disable “Allow affiliate registration” in Affiliates->Options, create a contact form, and create the affiliates request manually.
