After considering your last reply on July 4th regarding the missing password in the welcoming email, I have decided to skip Pending approvals for new affiliates, and set the default to Active. This has solved that problem.
Your CSS code fix also worked – thank you.
However I have encountered some other problems with notification emails:
Problem 1
Regarding the referral notifications, the “organization name” field is not populating in the administrator email, and the “organization name” and “first name” fields are not populating in the affiliate email (see screenshots at this link – https://www.dropbox.com/sh/024kg5noawkg524/AADDQJ1XSKUClfKPLS0cU9A0a?dl=0)
Problem 2
Each of the administrator and affiliate referral emails are being sent twice for some reason
Problem 3
All notification emails that are sent to affiliates are coming from wordpress@thespectrummarketplace.com.au instead of mail@thespectrummarketplace.com.au.
Why is this so?
Can you check these out and advise me? (The administrator login user name and password I previously advised are still current if you need to take a look at the back end of the site).
On another matter, I notice in the documentation you recommend against using WP Optimise as a caching plugin.
I have tried quite a few of the recommended and popular caching plugins (W3 Total Cache, WP Super Cache etc) on sites I have built, and have found WP Optimise to be the most effective for site speed and general performance.
I get consistently better scores on tools such as Pingdom and GTMetrix with this plugin, and with 900,000+ downloads and over 1000 five star reviews it must have something going for it.
Is there any specific reason why I should not use it?

Good morning, and thank you for being patient.
Please permit me to start with your last inquiry.
If a user visits your site and makes a purchase a few days(<30days by default) after an affiliate link was used, the club who's referral link was used would still be credited. This is so because the cookie used to store the Affiliate info would not have expired.
However, if you would prefer to have the cookie expire sooner, for instance after the user closes the browser, you could set the Referral timeout to 0 days.
If you would love to use a later date, then adjust the days accordingly.
With respect to question 1 and 2, please email logins to the site one more time eugen [at] itthinx [dot] com, so I can take a closer look.
NB! The logs didn’t pick up any weird issues related to Affiliates, so I would need to take a closer look at it.

The fields that are not displaying are definitely entered correctly. Thankyou for that solution. I have installed the CB Change Mail Sender plugin.
There is another thing that is troubling me.
I understand that if a member of one of my affiliate sports clubs makes a purchase after visiting my site via a banner or link that I have provided to them, then their club will receive a commission.
However, what happens if that member visits my site a few days later by merely searching for it on the net and then makes another purchase? Does their club still receive a commission on that purchase even though they did not come to the site via an affiliate link on that occasion?
I can see this happening a lot. Even though a member might use the original affiliate link from their club website or newsletter, once they know of my site they will probably just visit it directly.
Does the Referral Timeout setting have anything to do with this? Should it be set to a very long period? (I have seen 32000 days suggested in other articles I have read).

This is very odd. Please could you enable WP Debugging and share the debug log? Just so I don’t clutter this thread, please have a look at this enable WP Debugging.
Question 3:
Please use the CB Change Mail Sender
Ideally, they minify JS without checking if its already minified and this is usually a big problem with extensions. And could be a cause to many minor and major WP related issues.
