Affiliation sign-up form

I need a way to custom make a affiliation sign-up form, so when someone signs up they are asked for extra information, such as name, tax id, address, etc.

    There also is Affiliates Gravity Forms, note that it requires a Gravity Forms developer license for the User Registration Add-On.

    Hi Bill, Affiliates Extra Fields provides a solution for that.
