Affiliates Sign Up form not working

Hi, I have created my Affiliates Sign Up form, however upon testing I am unable to proceed after filling up the form and clicking on “Sign Up”…nothing happens after that. Please help urgently. Thank you!

    Also, I have set the affiliate parameter name as signup

    Once you have managed to fix the affiliate signup form, please use that affiliate ID to access the URL http://altsreport.com/signup (followed by the referral ID of the affiliate that you had just signed up). That doesn’t seem to be working etiher.

    Please get back to me regarding both issues.

      The form is not sent. You have a javascript error that is probably the reason ( http://oi67.tinypic.com/vdptaq.jpg )
      Probably another plugin or your theme is changing the operation of the form. If you can, create a staging site where you can disable the other plugins, enable them one by one to find the problem.
