Affiliates Registration Widget not displaying

Hi,

I am trying to add the Affiliates Registration widget to my sidebar and it is not showing up. I have tried everything and it’s just not working.
Help? Am I missing something?

    I can add the widget but it isn’t showing up on the front end of my website.
