Affiliate’s Registration Form Not Showing Field Names

I have an affiliate’s registration page, and it previously worked properly. Now, suddenly there are no field names. Only the lines where the “name, email, etc” should all be input. But no one is able to sign up because they have no idea what the should be entering! Please help!!!
