Affiliate registration form generates 404 error

For some reason, the [affiliates_registration] shortcode is generating a form that posts to a 404 page.

Actually nevermind, I found the issue. Apparently I accidentally created/named an empty subfolder the same name as the page slug. So mydomain.com/affiliate was both a page in WordPress and an actual folder name. smh
